Athletic Paperwork & Mental Health

Athletic Forms

Important Fall 2026 Athletic Dates:

  • Athletic Paperwork is Due: August 14th
  • First day of Fall Sports Practice/VB Tryouts: August 17th 
  • Fall Athletics Parent Meeting: August 20th
All athletic forms must be completed in order to practice. All forms except for the physical can be completed online. ***THE PHYSICAL IS NEWLY UPDATED. PLEASE ONLY USE THE PHYSICAL DATED 09/2025 IN THE BOTTOM CORNER. It is possible that your physicians will not have the accurate copy so please take a copy of the paperwork with you.2026 KSHSAA Physical Form

What Do I Need To Do For Athletic Paperwork??

  • KSHSAA Sports Physical (Dated after April 1) - 5 pages in length

  • Skyward - Student Online Form for Athletic Participation (found in  STUDENT Online Forms)

  • Skyward - Parent Online Form for Athletic Participation (found in PARENT Online Forms) 

  • You must have paid your Catastrophic Insurance &  Activity Participation Fee

Paper physicals will not be accepted at the school. Parents are required to upload them to BOUND. 

We are using the Bound platform for all athletic registrations. Please follow the steps below to ensure your student is cleared for participation:
